MAOFP Family Physician of the Year Award

This prestigious award that recognizes the outstanding Family Physician, an osteopathic physician who has demonstrated exceptional commitment to promoting osteopathic family medicine in the state of Michigan. Nominees for this award should be a member in good standing of MAOFP and exemplify the highest qualities of dedication to the osteopathic profession, to patients, and to the community through professional and public service as a practicing physician. Scholarship and leadership activities in hospital, state society and national organizations will be recognized. The award winner will be invited to attend the MAOFP Summer Family Medicine Update to receive a plaque and congratulations during the President’s Banquet and Award Ceremony. Please submit the nomination form, a current Curriculum Vitae for the nominee and a supporting statement of accomplishments by March 31, 2017Extended to April 15th

MAOFP Distinguished Service Award

This award is given to an individual(s) or organization(s) exemplifying outstanding dedication to the profession by contribution to the advancement of osteopathic family practice in the state of Michigan. The nominees for this award should demonstrate the highest qualities of professional and public service that have positively influenced osteopathic family medicine. The honoree(s) need not be an osteopathic physician. The award will be presented only to a deserving candidate(s). The award winner(s) will be invited to attend the MAOFP Summer Family Medicine Update to receive a plaque and congratulations during the President’s Banquet and Award Ceremony. By March 31, 2017, Extended to April 15th please submit: Curriculum Vitae (CV); Reasons for the Nomination with supporting statement of accomplishments, i.e. Professional Activities, Community Activities, Scholarship and Leadership Activities.

Resident of the Year Award

The award winner will be invited to attend the MAOFP Summer Family Medicine Update. At the President’s Banquet and Award Ceremony the winner will receive $100 and a plaque. The criteria for this award will be based on the nominee’s strengths in the following areas: Exhibiting leadership qualities and community involvement during educational training (e.g. chief resident, chief intern, volunteering for organization of community projects, etc.); Exemplifying the highest quality of dedication to the osteopathic profession (e.g. teaching, involvement in political action/committees, etc.); Promoting osteopathic family medicine in the state of Michigan; and exhibiting scholarship achievement in the field of family medicine (e.g. research, submitting resident scholarly activity, etc.). Each candidate must submit a one-page biography and curriculum vitae, as well as a written statement(s) from each nominating person. The resident may have more than one nominating individual. Please be sure to include the contact information for the person(s) submitting a nomination and name of the resident being nominated.
