The Michigan Association of Osteopathic Family
Physicians (MAOFP) requests nominations annually for
our prestigious award that recognizes the outstanding Family
Physician of the Year, an osteopathic physician who has demonstrated
exceptional commitment to promoting osteopathic family medicine
in the state of Michigan.
Nominees for this award should be a member
in good standing of MAOFP and exemplify the highest qualities
of dedication to the osteopathic profession, to patients,
and to the community through professional and public service
as a practicing physician. Scholarship and leadership activities
in hospital, state society and national organizations will
be recognized.
The award winner will be invited to attend the MAOFP Summer
Family Practice Symposium & Risk Management Seminar to
receive a plaque and congratulations during the President’s
Banquet and Award Ceremony. Nominations must be received by
May 1st each year in order for consideration.

| The Michigan Association of Osteopathic
Family Physicians (MAOFP) is request nominations annually
for our prestigious Distinguished Service Award to be given
to an individual(s) or organization(s) exemplifying
outstanding dedication to the profession by contribution to
the advancement of osteopathic family practice in the state
of Michigan. The nominees for this award should demonstrate
the highest qualities of professional and public service that
have positively influenced osteopathic family medicine. The
honoree(s) need not be an osteopathic physician. The
award will be presented only to a deserving candidate(s)
and may not be awarded annually. |
The award winner(s) will be invited to attend
the MAOFP Summer Family Practice Symposium & Risk Management
Seminar to receive a plaque and congratulations during the
President’s Banquet and Award Ceremony. Nominations

The award winner will be invited to attend the annual MAOFP
Summer Family Practice Symposium & Risk Management Seminar.
At the President’s Banquet and Award Ceremony during the
seminar, the winner will receive $100 and a plaque. The
criteria for this award will be based on the nominee’s
strengths in the following areas:
1) Exhibiting leadership qualities and community involvement
during educational training (e.g. chief resident, chief intern,
volunteering for organization of community projects, etc.)
2) Exemplifying the highest quality of dedication to the
osteopathic profession (e.g. teaching, involvement in political
action/committees, etc.)
3) Promoting osteopathic family medicine in the state of
Michigan
4) Exhibiting scholarship achievement in the field of family
medicine (e.g. research, submitting resident scholarly activity,
etc) Osteopathic physician candidates for the Resident of the
Year award can be nominated by any of the following; a preceptor,
faculty member, mentor, DME, hospital administrator, physician,
or another resident.
Each candidate must submit a one-page biography and curriculum
vitae, as well as a written statement(s) from each nominating
person. The resident may have more than one nominating individual.
Please be sure to include the contact information for the person(s)
submitting a nomination and name of the resident being nominated. |

PRACTICE RESIDENTS, PROGRAM DIRECTORS & D.M.E.s
Program Directors and hospital affiliates please post this
announcement, and promote this MAOFP award to demonstrate
your successful training of our future D.O.s. Your institution
should receive credit for the hard work of your residents
and staff.
Every year the Michigan Association of Osteopathic
Family Physicians recognizes and bestows an award for the
outstanding scientific paper written by an Osteopathic Family
Practice resident in a Michigan training program. As part
of the requirements of an ACOFP approved residency program,
each FP resident must complete a scientific paper. MAOFP requests
submission of all scientific papers from FP residents in training
for review and consideration for the “MAOFP Family Practice
Resident Paper of the Year Award”. Previous publication
of the paper is acceptable. More than one paper may be submitted
for consideration, but only a single prize can be awarded
in a calendar year. MAOFP offers family practice residents
an opportunity to gain recognition and a monetary award for
their scholarly activity. First Place $500 Second Place
$200 Third Place $100 The winners will be invited
to attend our annual MAOFP Summer Family Practice Symposium
and Risk Management Seminar. The winners will be announced
and receive their award at the Stakeholders Membership Meeting
on Friday during the seminar. A condensed version of the first
place paper will be published in the winter issue of our bi-annual
MAOFP Newsletter. (Please note: this is a newsletter and
not considered an official medical publication therefore;
it does not limit you to having your paper published in any
other forum.) The MAOFP office must receive all papers by
May 1st in order to be considered for the award. Please forward
