MAOFP has different membership categories for different levels of stages of your career.  Select the level for you and the headline is for submitting a member application online. You can also submit an application via PDF form, printing and then mailing/faxing.

Complete Your MAOFP Active Membership Application Online

ACTIVE Membership – The Board of Directors may extend active membership to any graduate of an AOA approved College of Osteopathic Medicine and Surgery who is licensed to practice Osteopathic Medicine and Surgery in the State of Michigan and who presents a written application on such form and containing such information and further qualifications as the Board shall prescribe. The annual dues fee is $195 for active physicians, $97.50 for retired physicians and $50 for physicians in their first year of practice.

ACADEMIC Membership –  shall consist of students in AOA-accredited colleges of osteopathic medicine in Michigan pre-doctoral training programs and to residents in AOA or ACGME accredited Family Medicine post-doctoral training programs or fellowships in Michigan. These members shall be entitled to serve as committee members, board members and national delegates(with vote), but they may not hold office in the Association. They are not required to pay dues, assessments or educational seminar registration fees.

ASSOCIATE Membership - shall consist of qualified licensed osteopathic family physicians not currently practicing in the State of Michigan; graduates of accredited schools of medicine, dentistry, or podiatry professional activities involve specialized cooperation with osteopathic family physicians, such as qualified pathologists, radiologists, surgeons, or other medical professionals, who contribute to some phase of the special field of family practice, or who practice jointly with members of this Association; Doctors of philosophy, other healthcare-related disciplines, education, and other non-doctoral personnel holding teaching or research position in scientific fields in AOA-accredited healthcare facilities or colleges.  Associate Members are required to pay dues, assessments and fees as for Active membership.  Associate members shall have the same privileges as Active members, except they shall not vote, hold office or be nominated to serve as a delegate to the ACOFP Congress of Delegates.


ALLIED Membership – shall consist of individuals who hold a degree in health care profession who is currently employed with an Active member of the Association and contributes to the practice of that member.  The Allied member is not eligible for any other category of membership, and supports the mission and objectives of the Association.  Allied members pay dues, assessments and educational conference fees as determined and administered by the Board.  They shall not vote, hold office or be nominated to serve as delegates to the ACOFP Congress of Delegates.
